SUPPORT COORDINATION

A support coordinator assists you to understand how your plan can support you to achieve your goals, increase your independence and build your capacity.

 

The role of a support coordinator can assist you to achieve the following:

​

  • Support to understand and implement formal and mainstream supports and services

  • Assistance to find, link and coordinate suitable service providers and organisations in your community.

  • Develop service agreements and support to manage the budget in your plan

  • Support to navigate the participant portal and understand how your funding is managed

  • Resolve queries or concerns that may arise and develop goals for future plans
