SPECIALIST SUPPORT COORDINATION

Horizon Specialist Support Coordination services will support you to manage challenges in your support environment, we will work with you to reduce barriers to implementing or using your NDIS plan. Our Specialist Support Coordination services include all activities outlined in ‘Support Coordination’ however we also support you to address complex situations and barriers where appropriate. A Specialist Support Coordinator will be funded in your plan where there are additional high or complex needs in your situation.

 

Horizon Specialist Support Coordinators can work with you to achieve the following outcomes:

• Connect, coordinate and negotiate appropriate support solutions with multiple stakeholders
  to achieve well-coordinated plan implementation to resolve points of crisis

• Connect the participant with relevant supports and services (NDIS funded and mainstream)

• Ensure consistent delivery of supports with open communication

• Build the participants capacity to understand their NDIS Plan

• Design a service plan to meet the participants support needs, where appropriate

• Assist you to achieve your goals through building capacity, knowledge, resourcefulness and
  confidence

• Support you to access advocacy services

• Report to the NDIA
